Instructions to Parents

Congratulations on having an 8th grader preparing to graduate. This blanket form is for 8th graders who have participated in a Saint Rose sponsored activity during one of their 5th-8th grade years. To submit a blanket order, please complete the following form. Please double-check your child’s name and competitive activities to avoid us providing the vendor with incorrect information. Please submit your form by April 26, 2023. Each blanket will cost between $30-$40.
